Joint Affidavit of two disinterested persons who witnessed the administration of burial rites to establish the death of OFW (for claimant who belongs to the tribal/cultural communities)

Accident/Police Report

Proof of relationship to OWWA member (marriage certificate, birth certificate, CENOMAR issued by PSA)

Any valid ID card that will establish the identity of the claimant
